New Patient Information

Thank you for your interest in the Artisan Center.  Below are links to help us make your experience as efficient as possible.  Upon check-in, you will be asked to provide us with a form of identification (driver’s license or equivalent) as well as your insurance information.  Though not used for cosmetic procedures, having your insurance information facilitates our interactions with pharmacies and hospitals on your behalf.  The Artisan Center accepts personal checks, all major credit cards, and even offers financing options through Care Credit .
